With the support of the Oakland Innovator’s Award, MYS is providing three mini soccer camps during the school year, at no cost to parents and families. The second mini camp takes place on Jan.7, 2012 at Hoover Elementary for children ages 6, 7, and 8.  

Priority goes to returning MYS campers, ages 6, 7, and 8.    

To apply for MYS Mini Camp, download, print, and fill out the following forms:

1. MYS Mini Camp 2012 Application  ( required for NEW campers)
   
2. Release of Liability  (Required for returning campers AND new campers)
3. Standard Appearance Release  (Required for returning AND new campers)
   
4. Medical History Survey  (required for NEW campers)

Also, a photo copy of the camper’s insurance card, front and back, must be included.

Mail all forms to PO BOX 21055 Oakland, CA 94620 by Dec. 31, 2011 to be considered in the first round of applications.

NO ELECTRONIC VERSION OR FAXES ARE ACCEPTED.

We are enrolling 30 campers, ages 6, 7, and 8, this month with priority given to returning campers.  Parents will receive confirmation by email.
